This registration form is for practicing clinicians attending just the Workshop

• If you are signing up for the workshop AND conference (or just the conference), register through Gravens here.

• If you are a patient or patient advocate, you may apply to participate at a very discounted rate here.

• If you are an architect, engineer or other non-clinician you should register here.

Important - Donations:

1. There is an option to donate to the Sextant Foundation during registration. This is entirely voluntary.

2. The Zeffy platform does not charge Sextant any processing fees. However, it defaults to a suggested contribution to Zeffy. This is completely voluntary. If you don't want to contribute to Zeffy or would like to donate a different amount select other in the dropdown menu, then type in your desired contribution.
